This exam is about using a detailed fetal anatomy ultrasound to evaluate the developing fetus's structure and look for major congenital anomalies. Around 18–22 weeks the fetus has enough organ development for a thorough, systematic assessment of all major body systems—the brain and spine; the heart’s structure; the abdominal organs; facial features; limbs; and the umbilical cord and placenta. Measuring growth parameters and checking amniotic fluid and placental location help confirm gestational age and identify growth issues or placental problems, which can influence care planning.

While you might obtain information about fetal sex if the genitalia are visible, that’s not the primary purpose of this scan. The timing is chosen because this window provides the best opportunity to detect many significant anomalies early enough to counsel families and plan management, rather than timing delivery or solely assessing maternal placental function.
